Step 1

• Complete the Level II Background screening (click for instructions)

• Complete the Local Criminal Records Check (click for instructions)

Step 2

• Complete all mandatory certifications (click for instructions)

Step 3

• Complete two Professional Experience Verification forms (click for instructions and download form)

Step 4

• Submit copies of the following documents to docs@AdelCare.com OR bring them to our office at 3206 Conway Road, Suite 5, Orlando FL, 32812.

Our Credentialing Coordinator will contact you when the documents have been received.
